In order to facilitate your registration, please fill up the forms that apply to you and bring them to the Doctor's office on the day of your apointment. For all minors 12 years and older, please fill and sign the consent for treatment without the presence of a parent. For the Initial Visit a parent must be present. Children under 12 can not be seen without beeing accompanied by parent or a legal guardian.
